Work Permits Info

New for 2009

You now have to follow an online procedure

If you do not have a personal computer you may use one available in the guidance career

Go to Maryland Department of Labor

Here is the site www.dllr.state.md.us

Look under "Quick Links"
Click "Minor Work Permit"
Print hard copy

Here is the Permit Work Permit Application

Complete the "To the Minor"
Have your employer complete and sign the "To the Employee"
Have your parent/guardian sign and date the application after they review it
Fill out the Online Application completely using the data collected on the hard copy application. Save the information and print the Data Report

Take to SDHS Guidance

Hard copy of application	Hard Copy of Application
Data Receipt	Data Receipt
	Proof of Age: One of the following	Birth Certificate
		Driver's Licence
		State ID Card
